Where the second finger is defined as index or middle?
Thumb - Index - Middle - Ring - Little
... is the scheme I am most familiar with.
We should take this to another post really.

That is the accepted convention. The middle being the longest finger.

Bought this from International Magic a few days back. What a monster - it's thicker than two volumes of Card College! All seems v.advanced to a beginner like me, but nice to have something to aspire to. The sections on palming are great stuff and explanations are very clear, but my fingers aren't up to it yet! Still highly recommended though.

Patience. I would suggest you work slowly through it but not continuously. Concentrate on a move and try and stick with it until you have mastered it. Can be very frustrating but also worthwhile and satisfying.
