The easiest way to do the top change is hold the deck in mechanics grip.. hold the card in your right hand between forfinger and thumb at the bottom right hand corner. push off the top card off the deck..

now ONLY move your left hand and keep your right hand stationary.. this is the easiest and most deceitful way to do it I have found.. turn an little to your right as if to glance at something

It seems to me, that the pack in the hand can be thought of as an extension to the hand and the fact that it moves around with the hand in of little consequence to the spek. And so, of there is a good reason to move the pack hand, be it a gesture or a posture change, then this is all the spek cares about and the fact that the card (in the other hand) either stays still or moves in a fluid manner (as described in RRTCM) is all that's noticed.

Anneman describes a trick which involves holding the card in the right, taking the deck from the left and putting it down on the table and performing the sleight on the way.

Yes they all work but as you say, you can gesture with the hand with the deck in..

ir '' What card do you think you are holding'' and point with the left to their card.. while they glance you bring your hand back on the way do the move keeping the right hand perfectly still.. as they see the left hand moving all the time.. they wont suspect anything and by the time they glance up from looking at the back of the card they hold or whatever you direct their attention to, the move has been done.

Another subtelty i use is when the change happens i continue with my left hand and move forward away from the card in the riht hand and turn the hand downwards.. so it seems that my hand was always facing down, reducing the possibility it could have been involved in any type of switch.
