The Effect
Acme Portable Hole
The performer shows a deck of cards to a volunteer. The spectator is asked to think of one of the cards.
The performer draws a black "portable hole" on the black of a playing card using a marker pen. The magician suddenly peels off the hole and "throws" it into the deck, which he is riffling.
The performer asks the spectator to name the card. The magician searches through the deck and there is the named card with a largish (about the size of an old 10p piece) hole cut right through it.
The card may be handed to the spectator. The rest of the deck can be shown and look, no holes can be seen in the backs of any of the other cards. Only the spectator's choice is perforated.

I'd put it at a 2, I think. There's one slightly tricky bit (and only slightly).
Review
Very good overall. It's an unusual trick, with a memorable revelation. The "portable hole" bit is clever, although not absolutely necessary to the working of the effect. You could come up with another explanation for how the hole gets into the chosen card. The original idea might be a little too cartoony for your tastes, but it's quite a silly ending anyway.
The card itself may be handed out, but not the rest of the deck (as this was specifically asked).
It would reset fairly quickly.
Overall
It's a nice effect, but maybe not a total jaw-dropper. I'd say 9/10.
