Presentation is an important aspect of, especially on stage where you have to communicate with a large crowd of people.
I saw Max Somerset once perform at a Cabaret night in Soho and he did the routine where you milk someone's arm using a funnel.
The patter, presentation and involvement with the specs was so superb that he managed to make one simple effect last for 20 minutes. Light-hearted banter with the partcipants kept the audience amused and the whole performance and Max's personality were what made it entertaining rather than the effect itself. A good entertainer can make the most basic of effects become one of the most entertaining things the audience has seen. Karl/Supermario is right in saying that presentation and humour are the keys to stage success.
I like to put myself in the mind of the audience. Things that cannot be seen are only interesting to those participating on stage. With mentalism you can have more of an effect on the audience as you can involve them to a greater or lesser degree.
They key is picking the right effects for stage. I tried SAW once, at a smallish club and it couldn't really be seen (It's a section where I talk about Victorian circus acts). I replaced this with skewer through tongue and then through thumb which had more of a response due to the size and nature of the props used.
Like everything, it's trial and error and you learn by either trying something out amongst your sure-fire winners or practicing at for friends and working out the furthest distance it can be seen it.
