This is easily achievable depending on your skill level, but you'll have to be a bit more specific if you want helpful advice.
You've said that the two methods you have aren't suitable for the effect, but you haven't said why they aren't - this makes it impossible to give you advice on how to achieve what you want.
Personally I'd either perform a cover pass/add-on pass or I'd use a shuffle control to retain the top packet and just strip an X card on top of the controlled packet, but let us know (without exposure, of course!) the specifics and we can help a lot better.
As an after-thought, if you don't require the spectator to actually handle the selection, you can force a double, then by placing it on top it is automatically second from top...
