The absolute best way to get a TT that fits is to go to a shop and peruse their TT selection to find one that fits. Or, as has been suggested, go to conventions and look at the possible selections that may be on offer there.
I believe that it's Patrick Page who uses a WAY oversized TT. In his video on TTing, he explains how to use it properly, since you have to do a pinch move to secure the tip when stealing, rather than relying on a snug fit to do the job. Jumbo tips can be found in many of the cyber shops, you just have to be patient and look around...Or, try e-mailing Davenports and tell them that you are looking for an oversized TT (only if, of course, you are!).
Speaking to the positioning of the TT when putting it on the thumb: To be completely honest with you, it simply does not matter. The tip should never be seen. In fact, you should be able to work with the tip if it is neon pink or neon green, and never be caught. The fact that the hand with the gizmo can be openly shown empty (with proper technique, of course) is just a bonus of the TT and nothing more. Also, normally, when using the TT, it shouldn't even be on the thumb at all.
If, however, you don't want to get into all of that and you have a problem with the positioning which is calling attention to the TT, then I suggest that you hold something in your hand that will cover the TT, until you can get yourself into a spot where you can reposition the gizmo. In other words, if you are doing a trick that uses a bill, then hold the bill with the dirty hand, fingers to the front, TT to the rear, and it will hide the tip enough that positioning won't matter. The same with a silk, a cigarette, or whatever you're working with.
