Crazy Man's Handcuffs is relatively simple to perform - there is no need to "swing" the index finger at all. When performed correctly your fingers won’t need to move position. The penetrating band does most of the work.
In regards to making them stay close together, its just practice and holding the bands correctly - press them together by pulling the bands tightly and slowly pull them apart. If done correctly this part of the illusion will really sell the effect.
I am curious to know what source you learnt CMH's from? As the problems you are having are normally covered extensively in most resources.
